Preventative Health Challenge

through November 30, 2025

Anyone who completes any type of wellness screening by 11/30/25 is eligible to be entered into our prize drawings.

• For employees enrolled in Cigna, no action is needed...Cigna will provide the necessary information.
• For those not enrolling in Cigna, email proof of their screening to payroll@cretepa.com to be entered. 

Additionally, employees who are enrolled in any of the following supplemental insurance products offered by Crete may be eligible for a separate incentive if they submit a claim:

• Critical Illness Insurance
• Accident Insurance
• Hospitalization Insurance
